As the main drug in the complex therapy of the following diseases: acute lymphoblastic leukemia( including in theth);lymphogranulomatosis;malignant non-Hodgkin's lymphomas;neuroblastoma;Wilms' tumor;osteogenic sarcoma;reticulosarcoma;lymphosarcoma;Mushroom mycosis, Ewing's sarcoma;Kaposi's sarcoma;malignant melanoma;rhabdomyosarcoma;sarcoma of bones and soft tissues, cervical cancer, breast cancer;small cell lung cancer, renal pelvis and ureter cancer, bladder cancer and children's gynecological tumors, solid tumors in adults.
How to use Vincristine-Mile
The drug is given only intravenously. The dose is set strictl
Adults are prescribed in a dose of 1-1.4 mg / m2 once a week. The maximum single dose is 2 mg / m2.The course dose should not exceed 10 - 12 mg / m2.
For children: Vincristine-Mili is prescribed in a dose of 1.5 mg / m2 once a week.
The course of treatment is 4-6 weeks.
Patients with impaired liver function should be given a drug in smaller doses.
Preparation of a solution. The solution is prepared immediately before use. The content of the ampoule is diluted with 0.9% sodium chloride solution to obtain a vincristine concentration of 0.1 mg / ml. Enter for 1 min.in a vein. The solution can be administered during the infusion of saline into the tube of the infusion apparatus. Vincristine can be used only by doctors with experience of chemotherapy.
With care, Vincristine is prescribed for elderly people, patients with concomitant neurological diseases or for radiotherapy in the area of the spinal cord due to the increased risk of neurotoxic action of Vincristine.
Patients with liver disease need a correction of the Vincristine dosage regimen. During the treatment it is necessary to control the picture of the blood.
The drug penetrates poorly through the blood-brain barrier, therefore, in the treatment of leukemia with neiroleukemia, other anti-tumor therapies may be necessary.
The patient should inform the doctor about the appearance of signs of an infectious disease.
Avoid accidental ingestion of the drug into the eye, because Vincristine can cause ulcers on the cornea.
Avoid extravasation with intravenous administration, as tissue necrosis is possible. In such cases, the injection should be discontinued and the drug that remained, enter into another vein. Locally apply heat and injections of hyaluronidase.
When treating Vincristine, women should avoid conception.
Side effects of Vincristine-Mile
From the side of the nervous system: paresthesia, seizures, ataxia;peripheral paresis;defeat of cranial nerves, headache, ptosis, hallucinations, sleep disturbances, peripheral nerves neuritis, depression, diplopia. Perhaps an exacerbation of concomitant neurological diseases.
On the part of the gastrointestinal tract: nausea, vomiting, diarrhea, stomatitis, abdominal pain, intestinal paresis, paralytic ileus, constipation, anorexia, elevation of hepatic enzyme levels.
On the part of metabolic processes: azotemia associated with dehydration;hyponatremia - as a consequence of abnormal ADH secretion.
On the part of the hematopoiesis system: very rarely - thrombocytopenia, anemia, leukopenia.
From the urinary system: atony of the bladder;polyuria, edema, dysuria, acute urinary nephropathy.
From the cardiovascular system: arterial hypertension;hypotension, tachycardia, arrhythmia, phlebitis.
On the part of the respiratory system: dyspnea, bronchospasm.
From the skin and its derivatives: at the site of erythema, burning sensation, rarely - local necrosis, alopecia.
Allergic reactions: anaphylactic shock, angioedema.
Other: weight loss.
Who is contraindicated in Vincristine-Mile
Disturbance of bone marrow function;leukopenia, diseases of the nervous system;bacterial and viral infections, pregnancy and the period of breastfeeding, hypersensitivity to the drug.
Interaction of Vincristine-Mile
With concomitant administration of Vincristine with allopurinol, the risk of myelodepression increases.
With simultaneous administration of Vincristine and other neurotoxic drugs, their nontoxic effect is mutually reinforced.
When mixing Vincristine and Furosemide solutions, immediate precipitation occurs.
When applying Vincristine with phenytoin, the antivascular effect may decrease.
When used in combination with mitomycin, severe bronchospasm may occur.
Symptoms: seizures, paralytic intestinal obstruction;complications associated with inadequate secretion of antidiuretic hormone.
Treatment: anticonvulsant drugs( phenobarbital), antibiotics( for suspected neutropenia).Limitation of fluid, the use of "loop" diuretics, laxatives. Control of the state of the cardiovascular system and peripheral blood. If necessary - substitution blood transfusion.
